Searching for a Physician in the Anthem Networks
- Go to Anthem's Web site
- Select Physician's/Facility
- For "Applicable States", select Indiana, Ohio
- Click on the Search the Provider Directory
- Select either quick searech or guided search option
How to use the Quick Search option
- Select the applicable plan
- Blue Access for the PPO Plus or PPO $900 Deductible
- Blue Preferred for the Blue Preferred Primary POS
- Select the type of Physician needed from the list provided
(for list of other specialists, use the Guided Search option)
- Type in the area to be searched:
- City of ZIP Code
- Mileage you are willing to travel
- Click on the Search button
- Browse list until you find the information desired
How to use the Guided Search option
- Select the type of assistance needed from the list provided
- Click on the Search button
- Type in the following information:
- Select a Healthcare Plan / Network
- Select a specialty
- Enter a location
- Click on the Search button
- Browse the list until you find the information desired
How to search for a Physician in the M-Plan
- Go to their Web site
- Select "Physician directory" or "Physician Directory for Guests" if you are not currently enrolled in M-Plan
- Choose a health network from the drop-down list
(Cardinal Health Alliance is Muncie; some in Indy; must be less than 50 miles from the doctor you choose.)
- If you are looking for a specific Doctor, enter his/her name
- Enter mileage you are willing to travel, state, and ZIP code
- View the list of Doctors available; Contact the Doctor to be sure he/she is taking new patients
- Record the Doctor's ID# and Network ID#. Both need to be included in your application
